Folger copy lacks first leaf (with preface) and last leaf (blank); some worm holes in pts. of the book, slightly affecting text. Bookstamp "Bibl. Publ. Basileensis" on leaf [A]2r; handwritten shelfmark on same leaf "G. XXI: 28"; number "42" handwritten on front pastedown in brown ink; purchased in Feb. 1902 (no. 670 in the Anderson sale catalog of John Appleton Library, Feb. 1902). In a contemporary pigskin binding blind tooled with fillet borders and a saltire in central panel on both covers; remains of 2 clasps catching on front cover; manuscript fragments used as spine reinforcement and inside the gatherings.
